Complete Application: Supporting Documents
Please read the following instructions carefully:
Applicants must submit supporting documents to verify their responses in their application. Responses that are not verified will not receive any points. Below are the types of documents that are recommended for submittal. Applicants should use their discretion to decide which of these and/or other documents to submit. All documents must be submitted electronically via this online application system. Do not mail any documents to WSLCB or the social equity contractor.
How to complete your application and uploaded supporting documents:
After submitting all of your supporting documents, click the "Complete My Application" button to complete and submit your application.
How to submit business owner information to your application:
Business Entity Applications require Business Owner Forms to be submitted for each owner of the Business Entity.
Click on the "Business Owner Form" button to complete and submit business owner information.
How to upload documents to your application:
For each section of supporting documents that you upload, please follow the step-by-step instructions in the below link (Document Upload Guide). You must create a folder and specify the name of the applicable section. For example, if you are uploading your TPI Verification documents, you should create a folder called "TPI Verification" and upload your documents into this folder. If you need assistance, please click the below link (Document Upload Guide). Please be reminded that application responses that are not verified with supporting documents will not receive any points.

True Party of Interest (TPI) Verification (Required Document)
Create a new folder titled “TPI Verification” and submit any one of the following documents below to verify the TPI.
Below are documents that prove TPI Verification.
Federal, state, or local government issued identification card showing applicant’s name
Government employee ID card (state or federal) with signature and photograph
Professional license (i.e., nurse, physician, engineer, etc.)
Veteran Administration identification with photograph
Washington driver’s license
Residency Verification (Required Document)
Create a new folder titled “Residency Verification” and submit any one of the following documents below to verify your residency verification.

Bank loan documents that show the applicant’s address
Bank statements or credit card bills
Correspondence from the federal, state, or local government
Correspondence from the Internal Revenue Service (IRS) or Washington government agency
County tax notice
Deed or title to real estate residential property

Create a new folder titled “DIA Verification” and submit any of the following documents below to verify your time spent in a DIA.

TPI family member arrest record (must include an affirmation of the familial relationship signed by the applicant and the family member)

Household Income Verification
Create a new folder titled “Household Income Verification” and submit any of the following documents below to verify your household income.

Copies of their most recent tax return
Documentation from the Office of Unemployment on their statements of benefits
IRS tax form W-2
Most recent federal tax return
Pay stubs that show the applicant’s name and Washington address
Student identification or confirmation of secondary education status
Conviction Information Verification
Create a new folder titled “Conviction Information Verification” and submit any of the following documents below to verify your conviction.
Below are documents that prove Conviction Information:
Abstract of a record of conviction prepared by the court in which the conviction was entered, or by a state official associated with the state’s repository of criminal justice records, which indicates the following: the charge or section of the law violated; the disposition of the case; the existence and date of conviction; and the sentence
Criminal history background reports
Docket entry from court record that indicate the existence of a conviction
Minutes of a court proceeding or transcript of a hearing that indicates the existence of a conviction
Other court document related to the conviction or vacated sentences
Probation documents
Probation report
Record of judgement and conviction
Record of plea, verdict and sentence

Business Ownership Verification
Create a new folder titled “Owner Verification” and submit any of the following documents below to verify your Business Ownership.

Previous Cannabis Business Experience
Create a new folder titled “Cannabis Experience” and submit any of the following documents below to verify your cannabis experience.

Business Plan that identifies the member in the charter
Documentation from local jurisdiction
Organization Documents for Corporation: Articles of Incorporation; By-laws' Shareholders’ Agreement; Stock Certificate

Documentation Showing that TPI or TPI’s Family Member’s Incarceration Prevented TPI from Obtaining Employment
Create a new folder titled "Employment Prevention" and submit any of the following documents below to verify that TPI or TPI's Family Member's incarcerations prevented them from obtaining employment.
Below are documents that prove Documentation Showing that TPI or TPI’s Family Member’s Incarceration Prevented TPI from Obtaining Employment:
Court record which shows that the TPI was terminated or denied employment due to the TPI or the TPI’s family member’s incarceration
Email from an employer which rejects or terminates employment and states the reason of the TPI’s rejection or termination was because the TPI was incarcerated
Letter from an employer which reject or terminates employment and state that the reason of the rejection or termination was because the TPI was incarcerated
Documentation Showing Loss of Home or Inability to Purchase Home as a Result of TPI’s Drug Convictions or Arrests
Create a new folder titled “Loss of Home” and submit any of the following documents below to verify loss of home or inability to purchase home.
Below are documents that prove Documentation Showing Loss of Home or Inability to Purchase Home as a Result of TPI’s Drug Convictions or Arrests:
Court record
Email from banks or landlords denying a mortgage or rental to the TPI because of the TPI's drug conviction or arrest.
Employment Termination Letter
Eviction Notice
Letter from banks or landlords discussing the reason for denial in mortgage or rental due to the TPI's drug conviction or arrest
Other Documents Not Listed
Applicants are permitted to upload additional copies of documents which may verify eligibility. Based on the date, credibility and source of the documents submitted, the social equity contractor will determine the acceptability of documents that are not listed above.
